Having Medicaid or Extra Help does not create one unlimited monthly Medicare Advantage switching right. The Dual/LIS SEP and Integrated Care SEP allow different actions for different groups.

This guide answers when and how a D-SNP enrollment or coverage change may be allowed. For the definition, eligibility categories, benefits and Medicare-Medicaid coordination, start with What Is a D-SNP?

Dual/LIS SEP

Allows a dual-eligible or LIS-only person to elect a standalone Part D plan in any month. It can also support returning from Medicare Advantage to Original Medicare with a standalone PDP. This SEP is not available while the individual is identified as a potential at-risk beneficiary or at-risk beneficiary under a Part D drug management program.

Integrated Care SEP

Allows a qualifying full-benefit dual member to enroll in an eligible integrated D-SNP in any month when the enrollment aligns Medicare and Medicaid coverage as required.

Neither SEP is a blanket monthly pass to move into any ordinary Medicare Advantage plan or freely switch among non-integrated MA plans.

Part D drug-management exception: The monthly Dual/LIS SEP is not available while Medicare identifies the beneficiary as a potential at-risk or at-risk beneficiary under a Part D drug management program. The Part D plan sends written notice when this designation applies. This restriction is specific to the Dual/LIS SEP and does not by itself eliminate every other Medicare enrollment period that may be available.

Initial enrollment

A person first eligible for Medicare may choose coverage during the applicable Initial Enrollment Period.

Annual Enrollment Period

From October 15 through December 7, eligible beneficiaries can generally join, change or leave Medicare Advantage or Part D coverage for January 1. Review the Annual Enrollment Period guide.

Medicare Advantage OEP

From January 1 through March 31, a person already in Medicare Advantage generally has one opportunity to switch MA plans or return to Original Medicare. Review the Medicare Advantage OEP guide.

Event-specific SEPs

Moving, losing Medicaid, gaining Medicaid, a plan termination or another qualifying event can create a separate SEP with its own dates and allowed actions.

Five checks before using a D-SNP SEP

  1. Confirm current Medicaid status. Full-benefit, QMB-only, SLMB-only and other categories do not produce identical plan eligibility.
  2. Name the action. Joining an integrated D-SNP, switching a PDP and returning to Original Medicare are different transactions.
  3. Verify the exact D-SNP. CMS publishes an annual list of integrated D-SNPs eligible to receive enrollment under the Integrated Care SEP.
  4. Check alignment. The Medicare D-SNP and Medicaid managed-care enrollment may need to be affiliated or otherwise aligned.
  5. Coordinate medical and drug coverage. Avoid an unintended gap, duplicate drug coverage or automatic disenrollment from incompatible coverage.

Why plan comparison still comes before enrollment

An SEP answers whether Medicare permits a change; it does not answer whether the new plan is a good fit. Confirm provider networks, hospitals, prescriptions, pharmacies, prior authorization, Medicaid coordination and supplemental-benefit restrictions.

Can every dual-eligible person switch D-SNPs monthly?

No. A monthly Integrated Care SEP may allow a qualifying full-benefit dual member to join an eligible integrated D-SNP when alignment rules are met. Other changes require another valid enrollment period.

What does the Dual/LIS SEP allow?

It allows a person with Medicaid or Extra Help to elect a standalone Part D plan in any month. It may be used when returning from Medicare Advantage to Original Medicare with PDP coverage. Does losing Medicaid create a different SEP?

It can. Loss of special-needs status and loss of Medicaid or Extra Help can create separate enrollment rights. The timing depends on the notice and the individual situation.
